  To help you manage your insurance plan, we have created a list of valuable resources to help answer some of your most frequent questions.
 

ID Cards

If you need a replacement or new ID card you can either call the appropriate carrier or you can log on their website.  Please see your plan administrator for the contact information.  If you know the name of the insurance carrier you can use the below link under the carrier customer service tab.

Claim Issues

  1. Check to make sure you’ve received an Explanation of Benefits (EOB) from the carrier.  If you have received an EOB, review it and read any notations at the bottom. 
  2. If you feel the claim has been incorrectly processed, please call the customer service number on the back of your ID card.
  3. If you have not received an EOB and have received a bill from the provider please contact your provider and make sure that they have the proper insurance information and fax them a copy of your current ID card.  If they have not billed your insurance it will take approximately 30 -45 days for your claim to be processed and for you to receive the EOB.

If after you’ve taken the above steps this does not resolve your claim issue, please do not wait to hear from a collection agency before contacting our office for assistance.  You can call 623-322-1647??? or email us at service@ibgaz.com.

RX Reimbursement

If you’ve paid for a prescription out of pocket you can request reimbursement from your medical carrier.  To get the proper instructions for reimbursement you should call the customer service number of on the back of your ID card.  Please keep in mind it will take 30-45 days to receive reimbursement after the claim is submitted.

Address Change

If you need to update your address with any of the insurance carriers please contact your plan administrator to make the change.
